Welcome to COTC’s Nursing Programs
Congratulations on your acceptance into this life-changing program where you’re on your way to a fantastic career. To maintain your seat in the program, you must complete several tasks. Please click the links below to view instructions and materials for each item.

Instructions
- Log in to Student Planning to view your course timeline and class options. You will not be able to register for any nursing courses in Student Planning.
- Your required courses are listed out by an advisor under Student Planning > My Academic Schedule and Timeline > Timeline. They should match the appropriate plan of study, minus any general education courses you have already completed:
- Go to ‘Academic Schedule’ and then click ‘View Other Sections’ under each of the classes listed. Select your preferred section (days/times) for each course. Make sure there are no overlapping times for courses.
- Go to the Registration Form and select the appropriate options for each class. Your first section preference for each class can match the sections you picked out in Student Planning. However, you are required to put a second and third choice as well. There is no guarantee you will be placed in a preferred section.
- Students who would like to add general education courses that are listed past the first semester requirements of the program may request approval to register through Student Planning any time.
- Please take care of all account holds: financial/unpaid tuition, library, parking, etc.
- This includes New or Returning Student Orientation if needed. If you still need Student Orientation, you were already contacted via email. Please follow the instructions there or check your Canvas for the module, then complete.
- This also includes your Financial Responsibility Agreement, which must be completed every semester. View instructions on how you may accept the Financial Responsibility Agreement.
- We are unable to register your classes until these holds are resolved.
- If you are a conditional student, please take care of your condition to acceptance ASAP, and notify us when completed. We will be unable to register you otherwise.
Programs of Study will be updated on student accounts automatically by our Records Dept. Please allow one week after you’ve attend Orientation for processing.
Registration will be finalized by Friday, Dec. 5, 2025 by 5 p.m. Check Student Planning to view finalized schedule.

Complete background check.
All incoming nursing students must complete a new FBI and BCI background check per clinical policy by Friday, Nov. 21, 2025. Previous background checks will not be accepted.
We prefer students complete background checks through the Department of Public Safety. You must first pay the required $60 ($35 for FBI and $25 for BCI) fees online and complete and print the Webcheck Request Form.

Then, please take your completed, printed Webcheck Request form, photo ID and receipt to the the Department of Public Safety in the Warner Center at the Newark campus to complete your background checks. Please keep a copy of your receipt for proof of completion if asked for by the nursing department. Use code 4723.09 for “Reason Fingerprinted.”
Students may complete the background checks off campus as long as the results are sent directly to: COTC Records, 1179 University Drive, Newark, OH 43055. It is the student’s responsibility to make sure results are received.
Please retain your receipt for proof of completion by Friday, Nov. 21, 2025.

Student ID
You are required to get a COTC student ID. COTC student ID cards are issued by The Ohio State University BuckID system and are available locally at the Department of Public Safety on the Newark campus.
- Visit the Department of Public Safety in Newark in Warner 105, or you can have your photo taken at the extended campuses (Coshocton, Knox or Pataskala) and pick up your ID a few days later.
- You must show a photo ID to receive your COTC student ID.
- New students must be registered for classes before getting a COTC student ID.
Parking
Also, all COTC students, staff and faculty are encouraged to get a parking pass by providing the make, model and license plate number for their vehicle. Students may obtain a parking permit at no cost in the public safety office. Please bring your license plate number and student ID.
Student and visitor parking is available in any white-lined space on all COTC campuses. On the Newark campus, staff and faculty park in yellow-lined spaces.

Physical Requirements
You will be going to various clinical agencies throughout the program. All of these clinical agencies have specific requirements regarding the health status of the student. If you do not meet these requirements, you are not permitted to have patient contact at that agency; therefore, you would not be able to complete the program.
It is mandatory that you complete the required health status information prior to your first day of class. You are required to upload copies of your original documents to Viewpoint. You should keep the originals in a safe location of your choosing. The required documents are as follows:
- The Physical Examination Form completed by a physician or certified nurse practitioner, verifying there are no health-related reasons the student cannot attend the nursing program. If a certified nurse practitioner completes your physical, please provide the partner physician’s name. This document must be signed by the healthcare provider and dated within the past 12 months. Please use the COTC physical form when you make a physical appointment.
- All students must comply with the student health: immunization policy in the nursing programs student handbook.
- If you have questions about immunization recommendations, review the CDC’s Adult Immunization Schedule for healthcare providers.
- All students must maintain current certification in basic life support. The course you need is the Basic Life Support for Healthcare Providers (CPR and AED) provided by the American Heart Association. This is the only certification accepted and is a prerequisite for clinical assignment at all healthcare agencies with whom the college affiliates. The class is available through various healthcare agencies and is also available through COTC in course EMS-100. You must maintain up-to-date certification while you are in the program. You will be sent home from your clinical rotation if you are not in compliance.
These requirements are non-negotiable because the safety of our students and the patients we care for are our top priority.

Submitting Your Physical
Download nursing physical form
1. Collect all needed healthcare information, as well as your CPR e-card.
2. Activate your health portal Viewpoint account ASAP using the link for your cohort (program you are accepted in to).
- Be aware: The Viewpoint account is different than your drug test account. If you receive this message, “Contact your Administrator,” then you did not use the correct link.
- Check your spam or junk mail for the password.

3. Begin uploading any health information as you get it.
- Make sure your NAME is visible on each page and NO SOCIAL SECURITY numbers.
- When taking a photo to upload a vaccine or titer, include the entire page. Do not cut and paste.
- When saving your photo of each vaccine or physical page, use “save as” and name the file.;
- If you have gotten a physical elsewhere, you will need to write “See Attached” on COTC’s provided form, then upload the COTC form and the document from your actual physical.
4. When uploading your physical form to the physical tab, you need to upload ALL THREE PAGES! With your name at the top of each page and signature and date on the third.
