The mission of Student Life-Disability Services is threefold. We seek to ensure that all students with disabilities can freely and actively participate in all facets of college life. We provide and coordinate support services and programs to maximize educational potential and develop independence to the fullest extent possible. Finally, we strive to increase the level of awareness among all members of the campus community, so that students with disabilities are able to perform at a level limited only by their abilities, not their disabilities.
Our office arranges for reasonable accommodations and support services that are individualized and flexible. Disability Services creates opportunities for the same educational experiences, services, and enrichment to persons with disabilities that are offered to those who are not disabled.

Staff members at SL-DS are specialists in particular areas. You will meet with someone who is trained and knowledgeable, who will work with you on issues of academic accommodations, disability adjustment, learning strategies, advocacy skills, and transition. Staff members are involved in a variety of additional activities. At times they act as advocates for students to ensure access to appropriate accommodations or they may serve as consultants to faculty and staff on disability-related issues. They provide in-service training to the community as well as participate in various campus committees. The primary goal of each staff member, however, is to ensure that students with disabilities have equal access to our college and the education they seek.

Interprets documentation for students to determine extent of functional limitations according to ADA-AA and 504 guidelines. Determines academic adjustments. Develops policy and procedure in accordance with federal and state regulations and state of the art practices. Serves as program and budget developer and manager of Perkins Grant. Designated 504 coordinator for both institutions; assures compliance and coordinates periodic review audits.
Reviews and interprets documentation in accordance with ADA-AA and 504 of Rehabilitation Act to determine accommodations appropriate to functional limitations. Does student intakes and makes referrals to other departments and campus or community services as necessary. Collaborates with faculty to ensure that accommodation needs are met in the classroom. Assists with training students on adaptive technology.

Documentation Process and Recommendations:
Student Life-Disability Services (SL-DS) provides academic services and accommodations for students with diagnosed disabilities. The documentation provided regarding the disability diagnosis must demonstrate a disability covered under Section 504 of the Rehabilitation Act of 1973 and Title II of the Americans with Disabilities Act (ADA-AA) of 1990. The ADA-AA defines a disability as a physical or mental impairment that substantially limits one or more major life activities.
SL-DS will review and consider all pieces of documentation submitted. Documentation will assist SL-DS in understanding how the disability impacts the student in an academic setting and the current impact of the disability as it relates to the accommodations requested. Reviewing documentation is a collaborative process and each piece of documentation is handled on a case-to-case basis. The eligibility determination process is not a same day process.
Once documentation is received, it will be reviewed in order of receipt; therefore, it is recommended that documentation be submitted well in advance of any accommodation related needs. The student will be contacted by phone or an email to their COTC campus email address once the documentation has been reviewed. Upon receipt of documentationl the student is required to meet with a specialist for an intake appointment to be considered fully registered with the office. If you would like to discuss the documentation review process, it is possible to directly speak with an SL-DS specialist so your questions and concerns can be addressed.
There are multiple ways to provide SL-DS with documentation of your disability. This documentation should be completed or provided from a treating or diagnosing healthcare professional (psychologist, psychiatrist, counselor, therapist, social worker, medical doctor, optometrists, speech-language pathologists, etc.). The various methods are explained below:
A full detailed evaluation or diagnostic report would provide the richest information to determine the best possible accommodations and supports.
A report that provides proof of prior accommodations.

Documentation that illustrates past use of postsecondary accommodations
- SL-DS has provided a “Disability Verification Form” that can be completed by the healthcare professional.
A healthcare professional can submit a letter of their own. Please provide the following information to SL-DS on letterhead with the date and signature:
A diagnostic statement identifying the disability (with the date of diagnosis)
Severity of the disorder (mild/moderate/severe)
Medication or treatment currently prescribed
An assessment of major life activities that are affected by the impairment (concentration, memory, social interactions, class attendance, learning, reading, thinking, etc.)
Recommended accommodations that may assist the student in minimizing the impact of the condition in an academic setting (examples: test taking, class attendance, taking notes, understanding materials, speaking, comprehension, transportation, housing, etc.)
Include test scores when applicable
